Magnetic Resonance Spectroscopy (MRS) is a novel imaging technique that offers the opportunity for colocalising biochemical information relating to metabolites specific to the study of epilepsy with high resolution MRI. Aims: The work included in this thesis was undertaken with two fundamental aims. The first was to apply a standardised MRS methodology in order to gain reproducible semiquantitative information about the variation of relevant neuro-metabolites such as gamma amino butyric acid (GABA), glutamate (as glutamate plus glutamine [GLX]), N acetyl aspartate (NAA), myo-inositol (Ins) and creatine plus phosphocreatine (Cr) within epilepsy syndromes or pathological groups. The second main aim was to test a series of hypotheses relating to the regulation of the concentrations of these metabolites in the region of epileptic seizures, immediately following seizures and associated with particular medical and surgical treatment interventions.

Magnetic resonance spectroscopy (MRS) is a non-invasive technique useful both in research and neuroclinical evaluation. It relies on the same physical principles of magnetic resonance imaging providing information on chemical compounds in vivo. MRS uses the magnetic properties of several nuclei such as 13C, 31P and 19F, although the 1H is the most common due to its abundance and magnetic resona...

Ictal fear in temporal lobe epilepsy: surgical outcome and focal hippocampal changes revealed by proton magnetic resonance spectroscopy imaging.

BACKGROUND Ictal fear (IF) is most frequently associated with epileptic discharges from mesial temporal areas. OBJECTIVES To determine whether patients with IF were more likely to become seizure free after anteromesial temporal lobe resection compared with those without IF and whether they show more anteriorly pronounced metabolic changes assessed by means of multivoxel magnetic resonance spe...

Childhood temporal lobe epilepsy: correlation between electroencephalography and magnetic resonance spectroscopy: a case–control study

BACKGROUND The diagnosis of epilepsy should be made as early as possible to give a child the best chance for treatment success and also to decrease complications such as learning difficulties and social and behavioral problems. In this study, we aimed to assess the ability of magnetic resonance spectroscopy (MRS) in detecting the lateralization side in patients with Temporal lobe epilepsy (TLE)...

Mapping form and function in the human brain: the emerging field of functional neuroimaging in cortical malformations.

Malformations of cortical development (MCDs) are increasingly being recognized as a common cause of epilepsy in cases previously felt to be cryptogenic. MCDs occur when the normal process of cerebral cortical development is disrupted, and include disorders of neuronal proliferation, migration, and organization. Many have a genetic basis and the genes responsible for some MCDs have been identifi...

Seizure frequency and bilateral temporal abnormalities: a proton magnetic resonance spectroscopy of temporal lobe epilepsy

Proton magnetic resonance spectroscopy ((1)H-MRS) was performed in seven healthy volunteers and 17 patients with temporal lobe epilepsy (TLE) to clarify the correlation of the severity of epilepsy with bilateral temporal changes in N-acetylaspartate (NAA), choline-containing compounds (Cho) and creatine + phosphocreatine (Cr). Despite unilateral EEG focus, bilateral temporal reduction in NAA /(...

Metabolic Asymmetries as an Early Indicator for Temporal Lobe Epilepsy in Children. A Comparison with EEG and High Resolution MRI

INTRODUCTION High resolution magnetic resonance imaging of the temporal lobe shows most cases of mesial temporal lobe sclerosis, the most common cause of temporal lobe epilepsy (TLE). In combination with EEG it yields lateralizing results in approx. 90 % of the patients. Several clinical studies on adult patients as well as on children (1,2,3) have demonstrated that proton magnetic resonance sp...
